Print down the form , Click the bottom left icon . Translate it to english with google chrome for you to understand if your not chinese .

Click the left bottom 4 to download the customer form . When u've downloaded the form , inside the file will have 3 microsoft words click the 3rd one , the 3rd one is to manage your account ( password ,name,etc )
And sorry HK's customer service form doesn't provide english . So you will have to translate it all into english with google translator and than slowly fill in the forms with correct details and make sure to scan the front and back of your ic .
